Abstract

PROGRAM STUDI D III KEPERAWATAN FAKULTAS ILMU KESEHATAN UNIVERSITAS BAKTI TUNAS HUSADA Karya Tulis Ilmiah, Juni 2025 Faiq Dhiya Wahyu Implementasi Monitoring Saturasi Oksigen Pada Pasien Congestive Heart Failure (CHF) Dengan Masalah Keperawatan Gangguan Pertukaran Gas Di Ruang Melati 2B RSUD dr. Soekardjo Kota Tasikmalaya xiii + 64 halaman + 7 tabel + 20 lampiran ABSTRAK Latar Belakang: Gagal jantung kongestif ialah kondisi di mana jantung tidak mampu memompa darah secara efektif untuk memenuhi kebutuhan oksigen dan nutrisi jaringan tubuh. Agar dapat mengetahui keberhasilan pemberian terapi oksigen, perawat memiliki peran penting dalam pelaksanaan dan pemantauan terapi tersebut. Pemantauan ini perlu dilakukan secara ketat dengan menggunakan oksimetri nadi guna menjaga saturasi oksigen tetap dalam batas normal. Tujuan: Untuk mengetahui implementasi monitoring saturasi oksigen pada pasien CHF dengan masalah keperawatan gangguan pertukaran gas. Metode Penelitian: Deskriptif observasi dengan desain studi kasus. Hasil: Hasil pemantauan saturasi oksigen pada responden I hari ke-1, rata-rata SpO₂ sebesar 94,8% dengan nilai terendah 93%. Kemudian hari ke-3, rata-rata mencapai 97,5% dengan kisaran nilai saturasi oksigen mencapai 96–99%. Pada responden II hari pertama, rata-rata SpO₂ sebesar 93% dengan nilai terendah 91%, kemudian pada hari ketiga, rata-rata SpO₂ mencapai 96,5%, dengan seluruh nilai dalam rentang normal (≥95%). Kesimpulan: kedua responden menunjukkan tanda-tanda perbaikan klinis, terutama dalam aspek frekuensi napas, suara napas tambahan, dan penggunaan otot bantu napas. Namun, responden II masih menunjukkan gejala sesak napas yang konsisten, yang menunjukkan perlunya pemantauan lanjutan dan evaluasi. Saran: Peneliti selanjutnya disarankan untuk dilakukan penelitian dengan kolaborasi tindakan batuk efektif dan posisi semi fowler untuk meningkatkan saturasi oksigen. Soekardjo Hospital, Tasikmalaya City xiii + 64 pages + 7 tables + 20 appendices ABSTRACT Background: Congestive Heart Failure (CHF) is a condition in which the heart is unable to pump blood effectively to meet the oxygen and nutritional needs of body tissues. To assess the effectiveness of oxygen therapy, nurses play a vital role in its implementation and monitoring. This monitoring needs to be carried out strictly using pulse oximetry to maintain oxygen saturation within normal limits. Objective: To determine the implementation of oxygen saturation monitoring in CHF patients with the nursing problem of impaired gas exchange. Method: Descriptive observational study with a case study design. Results: The oxygen saturation monitoring results for patient I on the first day showed an average SpO₂ of 94.8%, with the lowest value being 93%. On the third day, the average reached 97.5%, with saturation ranging from 96% to 99%. For patient II, the first day's average SpO₂ was 93%, with a low of 91%, and on the third day, the average SpO₂ increased to 96.5%, with all values within the normal range (≥95%). Conclusion: Both patients showed signs of clinical improvement, especially in terms of respiratory rate, presence of adventitious breath sounds, and use of accessory respiratory muscles. However, patient II still exhibited persistent shortness of breath, indicating the need for further monitoring and evaluation. Suggestion: Further researchers are advised to conduct research in collaboration with effective coughing actions and the semi-Fowler position to increase oxygen saturation.
