Abstract
Demam Berdarah Dengue (DBD) merupakan penyakit infeksi virus yang membutuhkan tata laksana suportif dan pemantauan terapi yang ketat karena pasien berisiko mengalami trombositopenia, kebocoran plasma, perdarahan, serta perubahan kebutuhan cairan selama fase penyakit. Penelitian ini bertujuan mengidentifikasi kejadian Drug Related Problems (DRP), menggambarkan jenis obat atau terapi yang berkaitan dengan DRP, serta menganalisis hubungan kejadian DRP dengan karakteristik pasien berdasarkan output SPSS. Metode penelitian yang digunakan adalah observasional deskriptif-analitik dengan desain cross-sectional retrospektif menggunakan data rekam medis. Berdasarkan penelusuran data, terdapat 844 pasien DBD yang teridentifikasi dengan kode ICD A90 atau A91. Dari jumlah tersebut, 346 pasien (41,0%) teridentifikasi mengalami DRP, sedangkan 498 pasien (59,0%) tidak teridentifikasi mengalami DRP. Identifikasi DRP dilakukan berdasarkan klasifikasi PCNE v9.1. Pada pasien yang mengalami DRP, kategori yang paling banyak ditemukan adalah efektivitas terapi tidak optimal atau P1.2 sebanyak 221 kejadian (63,9%), diikuti masalah keamanan terapi atau P2.1 sebanyak 125 kejadian (36,1%). Terapi yang berkaitan dengan DRP meliputi terapi cairan infus seperti Ringer Laktat dan NaCl 0,9%, paracetamol, antibiotik seperti ceftriaxone, cefixime atau cefotaxime, azithromycin, levofloxacin, dan metronidazole apabila tercatat, serta NSAID seperti ibuprofen, ketorolac, diclofenac, asam mefenamat, dan aspirin. Hasil uji Chi-Square pada data 844 pasien menunjukkan bahwa lama rawat inap berhubungan bermakna dengan kejadian DRP (p=0,044), sedangkan kelompok usia (p=0,473), jenis kelamin (p=0,856), jenis penjamin pasien (p=0,959), dan jumlah obat (p=0,367) tidak menunjukkan hubungan bermakna. Temuan ini menegaskan pentingnya medication review, pemantauan terapi cairan, pembatasan penggunaan antibiotik tanpa indikasi infeksi bakteri, serta penghindaran NSAID pada pasien DBD, terutama pada pasien dengan lama rawat lebih panjang. This study aimed to identify Drug Related Problems (DRP), describe drug or therapy groups associated with DRP, and analyze the association between DRP incidence and patient characteristics based on SPSS output. This research used a descriptive-analytic observational method with a retrospective cross-sectional design based on medical record data. A total of 844 DHF patients with ICD codes A90 or A91 were identified. Among them, 346 patients (41.0%) were identified as having DRP, while 498 patients (59.0%) were not identified as having DRP. DRP was classified using PCNE v9.1. Among patients with DRP, the most frequent category was suboptimal therapeutic effect or P1.2, with 221 events (63.9%), followed by treatment safety problems or P2.1, with 125 events (36.1%). Therapies associated with DRP included intravenous fluids such as Ringer's lactate and 0.9% normal saline, paracetamol, antibiotics such as ceftriaxone, cefixime or cefotaxime, azithromycin, levofloxacin, and metronidazole when documented, as well as NSAIDs such as ibuprofen, ketorolac, diclofenac, mefenamic acid, and aspirin. Chi-Square analysis of 844 patients showed that length of stay was significantly associated with DRP incidence (p=0.044), while age group (p=0.473), sex (p=0.856), patient insurance/payment type (p=0.959), and number of drugs (p=0.367) were not significantly associated. These findings emphasize the importance of medication review, fluid therapy monitoring, restricting antibiotic use without evidence of bacterial infection, and avoiding NSAIDs in DHF patients, especially in patients with longer hospitalization.
