Abstract

ABSTRAK Analisis Efektivitas Biaya Amlodipin dan Kaptopril pada Pasien Hipertensi Komorbid Diabetes Melitus Tipe 2 SALSA FERLINA Program Studi S1 Farmasi, Universitas Bakti Tunas Husada Hipertensi dengan komorbid Diabetes Mellitus (DM) tipe 2 merupakan kondisi yang membutuhkan terapi jangka panjang serta biaya pengobatan yang cukup besar, sehingga diperlukan evaluasi efektivitas biaya untuk menentukan pilihan terapi yang efisien. Penelitian ini bertujuan untuk menganalisis efektivitas biaya penggunaan amlodipin dan kaptopril pada pasien hipertensi dengan komorbid DM tipe 2 di RSUD dr. Soekardjo Tasikmalaya periode Januari–Desember 2025. Penelitian ini menggunakan metode observasional dengan pendekatan retrospektif berdasarkan data rekam medis pasien. Analisis farmakoekonomi dilakukan menggunakan metode Cost-Effectiveness Analysis (CEA) dengan menghitung biaya medis langsung, Average Cost-Effectiveness Ratio (ACER), dan Incremental Cost-Effectiveness Ratio (ICER). Hasil penelitian menunjukkan bahwa penggunaan amlodipin lebih banyak dibandingkan kaptopril, yaitu sebanyak 24 pasien (75%) dan 8 pasien (25%). Rata-rata biaya medis langsung terapi amlodipin sebesar Rp179,964, lebih rendah dibandingkan kaptopril sebesar Rp296,768. Nilai ACER amlodipin sebesar Rp2,699 lebih rendah dibandingkan kaptopril sebesar Rp4,748. Hasil perhitungan ICER menunjukkan nilai -Rp28,010 yang menunjukkan bahwa amlodipin merupakan terapi dominan (dominant) karena memberikan efektivitas lebih tinggi (66,67%) dengan biaya lebih rendah dibandingkan kaptopril (62,50%). Berdasarkan diagram efektivitas biaya, amlodipin berada pada Kuadran II (dominant), sedangkan kaptopril berada pada Kuadran IV (dominated). Berdasarkan hasil analisis tersebut, terapi amlodipin dinilai lebih cost-effective dibandingkan kaptopril dan dapat dipertimbangkan sebagai pilihan terapi yang lebih efisien pada pasien hipertensi dengan komorbid DM tipe 2. Kata kunci: Hipertensi, Diabetes Mellitus tipe 2, Amlodipin, Kaptopril, Analisis Efektivitas Biaya, ACER, ICER ABSTRACT Hypertension with comorbid Type 2 Diabetes Mellitus (DM) is a condition requiring long-term therapy and substantial treatment costs; therefore, a cost-effectiveness evaluation is necessary to determine the most efficient treatment option. This study aimed to analyze the cost-effectiveness of using amlodipine versus captopril in hypertensive patients with comorbid Type 2 DM at Dr. Soekardjo Regional General Hospital (RSUD), Tasikmalaya, during the period of January–December 2025. An observational study design with a retrospective approach was employed, utilizing patient medical record data. Pharmacoeconomic analysis was conducted using the Cost-Effectiveness Analysis (CEA) method by calculating direct medical costs, the Average Cost-Effectiveness Ratio (ACER), and the Incremental Cost-Effectiveness Ratio (ICER). The results showed that amlodipine was used more frequently than captopril, with 24 patients (75%) receiving amlodipine and 8 patients (25%) receiving captopril. The average direct medical cost for amlodipine therapy was Rp179,964, which was lower than that for captopril (Rp296,768). The ACER value for amlodipine (Rp2,699) was lower than that for captopril (Rp4,748). The ICER calculation yielded a value of -Rp28,010, indicating that amlodipine is the dominant therapy; it provided higher effectiveness (66.67%) at a lower cost compared to captopril (62.50%). Based on the cost-effectiveness diagram, amlodipine fell into Quadrant II (dominant), whereas captopril fell into Quadrant IV (dominated). Based on this analysis, amlodipine therapy is considered more cost-effective than captopril and may be considered a more efficient treatment option for hypertensive patients with comorbid Type 2 DM.
